But if body fat is the only thing you look at, you are walking past the two measurements on the report that may matter most for how well you age. Bone density and visceral fat are silent, symptomless, and invisible in the mirror, and a DEXA scan quantifies both in about ten minutes.
What a DEXA Scan Actually Captures
DEXA stands for dual-energy X-ray absorptiometry. You lie still on an open, flat table while a low-dose scanner passes over you. The radiation exposure is very small, comparable to a few hours of normal background exposure. In a single pass, the scan reports four categories of information: fat mass, lean (muscle) mass, bone mineral density, and an estimate of visceral adipose tissue, all broken out region by region across your arms, legs, and trunk.
That regional breakdown is what separates DEXA from every consumer device. A smart scale gives you one whole-body guess. DEXA shows you where your muscle and fat actually sit, and whether your skeleton is keeping pace with the rest of you.
Visceral Fat: The Number You Can’t Pinch
Subcutaneous fat is the fat just under your skin, the kind you can pinch. Visceral fat is different. It is packed deep in the abdominal cavity, wrapped around your liver, pancreas, and intestines, and it behaves more like an active organ than a storage depot. It secretes inflammatory compounds and disrupts how your body handles insulin.
This is why two people at the same weight, and even the same overall body fat percentage, can have very different health trajectories. The person carrying more visceral fat sits at higher risk for insulin resistance, type 2 diabetes, and cardiovascular disease, even if they look lean in clothes. You cannot pinch visceral fat and you cannot see it in a mirror. You have to measure it.
The encouraging part is that visceral fat is highly responsive. It tends to be among the first fat to decrease with consistent resistance training, improved nutrition, and reduced refined-carbohydrate intake. Bone Density: The Silent Decline
Bone is living tissue that remodels throughout your life, but bone mineral density tends to peak in your late twenties and gradually declines afterward. For women, the drop accelerates sharply around menopause as estrogen falls. For men, the decline is slower but still real. The problem is that bone loss produces no symptoms. There is no ache, no warning. The first sign is often a fracture that should not have happened.
DEXA is the clinical gold standard for measuring bone mineral density. The result is expressed as a T-score, which compares your bone density to that of a healthy young adult. A T-score between -1.0 and -2.5 indicates osteopenia, the early, manageable stage of bone loss. Below -2.5 indicates osteoporosis. Catching a downward trend in the osteopenia range gives you years of runway to respond, while waiting until a fracture removes most of your options.
The response is not complicated, but it has to be deliberate. Progressive resistance training mechanically loads bone and signals it to maintain or build density. Adequate protein, calcium, and vitamin D supply the raw materials. Impact and load-bearing movement matter more than any supplement marketed for bone health. Muscle and bone are built by the same stimulus, which is one more reason strength training sits at the center of healthy aging.
Why These Two Numbers Belong Together
Bone density and lean mass decline on parallel tracks, and both accelerate with age and inactivity. Sarcopenia, the age-related loss of muscle, and osteopenia, the loss of bone, are closely linked because the same resistance training that builds muscle also loads and preserves bone. This is the real structural story of aging well: the difference between an independent, capable eighty-year-old and a fragile one is largely written in muscle and bone, decades earlier.
A DEXA scan lets you read that story while you still have time to change the ending. It turns “healthy aging” from a vague aspiration into two specific numbers you can track and influence.

A single scan is a baseline. The real value comes from comparison. We typically recommend a follow-up scan after several months of structured training and nutrition, because seeing visceral fat drop or lean mass climb on an objective report is both motivating and corrective. If a strategy is not working, the next scan shows it, and the plan changes before months are wasted.
